Tibet Entry Permit is issued by the Tibet Tourism Bureau. And you can only get it trough qualified travel agencies using the copies of your passports and visa and some other documents, and it is requested before you even get on the plane or train to tibet. Aliens' Travel Permit is issued by the police (Public Security Bureau, "PSB"). Usually you can apply for it once you arrive at Lhasa. For tour groups, your guide will ask you for the passport and TTB permit and submit it to the Foreign Affairs Section of PSB for the Travel Permit. It normally takes several hours and the cost is 50 CNY/person. As for the Mountaineering License, it is issued by Tibet Mountaineering Association. You’ll also need certain documents and money (seem to be 200 US dollars per person) to apply for it.
